STUDENT ENROLLMENT VERIFICATION
ONLY PERTH AMBOY RESIDENTS ARE ELIGIBLE TO REGISTER.
ONLY PARENT(S) OR LEGAL GUARDIAN MAY ENROLL A STUDENT.
REGISTRATION CHECKLIST:
1). PROOF OF DOMICILE (RESIDENCY)/4 CHOICES (CHOOSE ONE)
a). DEED, showing ownership of residential property in the district.
b). LEASE, in effect for the current school year or to begin within 60 days of the date of enrollment.
c). CONTRACT FOR THE PURCHASE OF RESIDENTIAL PROPERTY IN THE DISTRICT, showing
date of settlement within 60 days of the date of enrollment.
d). NOTARIZED AFFIDAVIT along with appropriate supporting documents.
A drivers license, tax, electric, gas, telephone bills, or voter
registration card Will NOT be accepted as the only proof of residence.
2). ORIGINAL BIRTH CERTIFICATE
3). PROOF OF CUSTODY FOR SEPARATED/DIVORCED
4). PARENTS/SINGLE/OR GUARDIAN formal court document demonstrating custody.
5). IMMUNIZATION RECORDS
6). CHILDS PRIMARY LANGUAGE (English, Spanish or ________)
