Student Opt Out Procedures

Pursuant to District Policy 5250 and N.J.S.A. 18A:35-4.7, a parent or guardian may request that their child be excused from any part of instruction in health, family life education, or sex education that conflicts with the parent’s or guardian’s sincerely held moral or religious beliefs by submitting a signed written statement to the school Principal.  

The District will consider other religious accommodation requests outside of health, family life education, or sex education on a case-by-case basis consistent with applicable law. 

Parents must submit their opt-out request in writing, directly to the building principal, using the official forms below: 

• Family Life/Health/Sex Education Opt Out Forms
• Dissection Opt Out Form
• Other Specific Topic/Lesson Opt Out Form

For parents who may need assistance printing the forms above, please contact the building principal, who will provide hard copies.
