All students entering Kindergarten and new students to the district are required by State

school health guidelines to undergo a physical examination. The physical should be

given at your “medical home” which is your pediatric, clinic, health center, etc.

Kindergarten Students

The physical must be completed prior to entering school. You may bring in or

send in the physical examination form to your child’s school nurse. If your child has had

a physical within the last eleven (11) months, that will be acceptable. Have your doctor

complete the attached form.

New Students

You will have 30 days in order to secure a physical for your child or he/she may

be excluded from school. If your child has had a physical within the last eleven (11)

months that will be acceptable. Have your doctor fill out the attached form and return the

form to the school nurse.

Todos los estudiantes que ingresan a Kindergarten y nuevos estudiantes al distrito
