The combined Parental/Guardian Consent & Photo Release Form is available for download and printing. Please use the top of this form to allow/deny permission for your child's photo/image and personally identifiable information to be published on the district and/or school's web site. Use the bottom of this form to allow permission of the Perth Amboy Board of Education to use a photograph of your child for school promotion purposes including reproduction in any of the Board's publications, posters or media or releases to newspapers or outside media, both print and electronic and/or your child's name in publicity releases and field trip information.
