    ***IMPORTANT REMINDER!***

    ***NJ State Immunizations: NJ State requires that your children's immunizations are updated and completed according to NJ State immunization requirements according to state regulations. Please send in all information requested to the school nurses office ASAP. It may be emailed to us at the email address listed above, faxed or mailed to the school.

    NJ Physical Examination forms can be found on this page under " Information & Forms " and, if requested, must be submitted to the school nurses' office ASAP

    ****Medical Concerns & Medications ***For parents who have a child with any medical concerns such as Asthma, Food Allergies, Seizures, Cardiac History, and Diabetes, ect. please obtain paperwork for your child to be able to receive child's health health and to update your childs Health records. Please send the complete order before school begins to the school nurse. (Ensure that parents' signature and physicians' signature are on all forms). All medication is due upon returning to in-person learning.

    Student Medication Pick-up: If you would like to pick up your children's medications that are in the nurses' office, please call 732-376-6072 or email us and we will respond promptly. The nurses will set a date and time for pickup. 

    Students with Asthma:

    ***Please be aware that at this time Nebulizer Treatment use for Asthma while in school will not be permitted.
    ***Please have your doctor order an Asthma Inhaler for use in school. Asthma Plans are found on this page under              " Information and Forms"
