Supplemental Educational Services (SES)
INTRODUCTION
If your child receives free or reduced price lunch, he/she may be eligible to receive free supplemental educational services (SES). If you choose to enroll your child, you will be able to select a program that best fits your child's needs. This notice will help you understand how SES can help your child and what you must do to take advantage of it.
1. YOU GET TO CHOOSE
One of the most important features of SES is that the parents are the ones who will make choices for their children. You will be able to choose among different types of services, given by different providers at different locations. That way, you can decide what is best for your child. But there is no obligation to enroll your child in SES.

2. THE SES PROVIDERS
A list of providers that have been approved by the state to offer services to your child within this area, a description of each provider, including the services they offer, their qualifications, and their performance can be obtained at the following locations or by visiting the NJ State Department of Education Website at http://www.state.nj.us/education.- Perth Amboy High School

3. WE MAY NOT BE ABLE TO ACCOMMODATE EVERY CHILD
There are only a limited number of spaces in the SES program so we may not be able to enroll every child whose parent applies. Final determination is dependent upon enrollment.
4. PRIORITY FOR LOWEST-ACHIEVING STUDENTS
In compliance with federal law, we will give first priority to the lowest-achieving students. The achievement level will be based on the students score on the statewide assessment program.
5. TRANSPORTATION TO YOUR CHOSEN PROVIDER
Our district does not include transportation to the provider.
6. APPLICATION DEADLINE
If you decide you want to enroll your child in the SES program and choose one of the providers, you must mail the attached form to us no later than October 31, 2011. Please mail the form to: Federal Office, 178 Barracks Street, Perth Amboy, NJ 08861.
