•Every form must be completed,signed, and submitted to the School Nurse/Athletic Trainer @ PAHS Main Campus.

•Your completed physical will then be reviewed by the school physician.

•Missing/incomplete forms will be returned and this will delay the process of participating on an athletic team.

To participate on a school-sponsored interscholastic or intramural athletic team or squad, each student whose physical examination was completed more than 90 days prior to the first day of official practice shall provide a health history update questionnaire completed and signed by the student's parent or guardian.  Please complete and return to the PAHS Main Campus Athletic Trainer/School Nurse PRIOR to summer practices.
